UDSv4 Follow-Up Visit Packets Now Available

UDSv4 FVP Available

We are excited to announce the launch of the UDSv4 Follow-Up Visit Packet (FVP) forms! Starting today, ADRCs can begin collecting and submitting follow-up visit data using these forms.

Update REDCap for the FVP

Please reference the REDCap project changes spreadsheet on this page to see changes that have been made to the REDCap UDSv4 project to incorporate follow-up visit packets. Updated REDCap files including the IVP and FVP can also be downloaded here.

NOTE: While the REDCap project changes spreadsheet is intended for use by Centers who will implement manual changes, all Centers are encouraged to review it to understand changes that have been implemented across the new REDCap files.

Updates to the ADRC Portal Sandbox Mode

Please note that the ADRC Portal sandbox mode now uses "test" identifiers of real participant identifiers. It is no longer possible to test submissions with real PTIDs. Please see the updated instructions on how to obtain a test identifier and submit test data in sandbox mode.
