Request to Use NACC Copyrighted Forms
The NACC data collection forms are widely regarded as the gold standard in longitudinal neurocognitive and clinical phenotypic data collection. The data in the NACC database is collected from Alzheimer’s Disease Research Center (ADRC) participants, but non-ADRC researchers can also request access to collect data for their own studies using the NACC forms, at no charge.
To obtain permission, please review the available data collection forms and complete a permission request describing the intended use of the forms you are requesting. Requests may take up to two weeks to process. (If you are interested instead in using already-collected NACC data, please submit a data request.)
