

We are thrilled to announce the launch of the Spanish UDSv4 Initial Visit Packet (IVP) forms, including the Neuropsychological Battery! Starting today, ADRCs can begin collecting and submitting initial visit data using these forms.
Note: The Spanish versions of the UDSv4 Follow Up Visit forms (FVP) and C2T (Neuropsychological Battery Scores for T-cog), as well as an update to the CLS, are targeted for release in Q1 2026. ADRCs can only do in-person or video neuropsych battery using C2 until C2T is available.
We want to extend our deep appreciation and thanks to the entire UDSv4 Latino Task Force!
This task force came together to develop a culturally and linguistically appropriate Spanish version of the UDSv4 so ADRCs can more effectively administer clinical and cognitive measures to participants from a wide variety of Latino backgrounds.
The Latino Task Force includes bilingual (Spanish/English) and bicultural experts in dementia from diverse Latino backgrounds who are affiliated with their institutions’ ADRCs and have access to Latino Spanish-speaking staff and populations.

Help us improve the Spanish UDSv4 forms!
We highly value your Center’s feedback and plan to incorporate as much input as possible from across the ADRC Program to improve the forms before their final release in Spring 2026. ADRC staff at all levels and across all functions are encouraged to submit feedback, and you may select one or more forms to comment on and submit feedback multiple times if needed.
Please submit all feedback by April 15, 2026. We will notify Centers when the updated forms are available next spring.
