
Collect non-ADRC Data
The National Alzheimer’s Coordinating Center (NACC) has granted permission to almost 300 researchers to use our data collection forms. Our forms have been requested to be used across X number of states and X number of countries.
The NACC data collection forms are widely regarded as the gold standard in longitudinal neurocognitive and clinical phenotypic data collection.
Most NACC data collection forms are available at no charge to investigators outside of the Alzheimer’s Disease Research Centers (ADRC) Program. To obtain permission, please review the available data collection forms and complete a permission request describing the intended use of the forms you are requesting. Requests may take up to 2 weeks to process.
